POSITION TITLE: RESIDENT CARE COORDINATOR

POSITION SUMMARY: Responsible for resident service plan and nursing staff development and duties as assigned. Duties include data entering personal care services, developing nursing staff monthly work schedules, coordinating orientation training for new staff and other tasks as needed for resident well-being.

REPORTS TO: HEALTH SERVICES DIRECTOR

POSITION RESPONSIBILITIES/ESSENTIAL JOB FUNCTIONS:

  • Develop and maintain resident service plan review schedule according to service plan policy and state regulation for timely reviews with family and/or responsible party.
  • Develop resident service plans to reflect accurate care services provided 24-hours a day by all departments.
  • Update database for move-in, hospitalizations, move-out and transfers
  • Upholds Residents’ Rights at all times.
  • Supports and upholds all company policies, care philosophy and mission statement.
  • Discusses with nursing staff any changes needing to be reflected in resident service plans. Changes in physical, emotional, mental, or behavioral condition, unusual symptoms, accidents, injuries, or unusual occurrences, and reports promptly to a licensed nurse.
  • Ensures most current updated service plan is available in a readily area for staff to provide care.
  • Coordinates with Administrative staff for placement of nursing staff job postings. Interviews staff for open positions within the nursing department.
  • Coordinate with Administrative staff for new employee orientation start dates to ensure new personnel are provided with the necessary training to meet the state regulations and company requirements.
  • Develop monthly nursing care schedule. Coordinate staff vacations, call-outs and any other time off needs.
  • Coordinate with Health Services Director monthly educational inservices for all staff in accordance with state regulations
  • Submit information for billings based on care point increases, decreases to accurate reflect care provided
  • Perform similar or related duties as required or directed.

EMPLOYMENT REQUIREMENTS:

  • Previous experience working with seniors in a residential or long-term care setting preferred.
  • Must have compassion for and desire to work with the senior population.
  • Must meet all state health requirements.
  • Ability to communicate effectively with residents, families, employees, vendors, and the general public. Adequate and clear English speaking and writing ability.
  • Ability to see well enough to read and write, and visually interact with residents, families and staff.
  • Keeps all information confidential.
  • Interacts in professional manner with residents, public, and employees. Works cooperatively with Management and other employees. Displays willingness to assist co-workers when situation requires or when asked.

  • Maintains acceptable attendance records/follows work schedule.
  • Organizes and utilizes time appropriately toward accomplishing assigned tasks.

  • Ability to exert 20 - 50 lbs. of force occasionally, and/or 10 - 25 lbs. frequently, and/or up to 10lbs. constantly to lift, carry, push, pull, or otherwise move objects.
  • Ability to bend, twist, squat and/or reach frequently and consistently while performing tasks.
  • Ability to spend long periods on feet and to ambulate quickly.
  • Ability to work safely and to recognize potentially dangerous situations.
  • Ability/willingness to follow all policies and procedures.
  • Is neat, accurate, dependable, and keeps work/space clean.
  • Ability/willingness to work longer than average eight (8) hour shift; work holidays or overtime; to work seven (7) day schedule.
  • Must be able to perform the essential duties and responsibilities with or without reasonable accommodation without causing a significant safety threat to self or others.

EDUCATION:

Twelve (12) years (High School Diploma or Equivalent) of education preferred. Attendance at mandatory in- service training. Maintain CPR/First Aid Certification.
